Nevada driving log form
Nevada has its own driving log, DLD-130. You fill in a row for each drive and turn it in.
In the state's own words
“All completed log sheets and a Certificate of Completion of a driver education course (if required) must be submitted prior to the driving skills test.”
“Beginning drivers under 18 years of age must complete 50 hours (3,000 minutes) of supervised driving experience in a motor vehicle as a condition of receiving a full driver license.”
“Ten hours (600 minutes) of this experience must be completed in darkness.”
“Signature of Notary Public or Field Services Representative”
Checked on the state's own website on October 18, 2026. Rules change, so confirm with your DMV before your appointment.
What this means for you
DLD-130 says the teen and the parent must keep a log on the back of the form. Page 1 holds the parent certification. Page 2 holds the drives and the totals. Copy page 2 if you need more rows, and bring every sheet before the drive test.
