Illinois driving log rules
In Illinois, a parent signs for the hours at the license office or the driving school. There is nothing to print at home.
In the state's own words
“Parent or legal guardian must accompany teen to provide written consent to obtain a driver’s license, OR complete and notarize an Affidavit/Consent for Minor to Drive form.”
“Parent or legal guardian must certify that a minimum of 50 hours of practice driving, including 10 hours at night, has been completed.”
“This chart is designed solely for your convenience to ensure your teen has completed the required 50 hours of practice driving.”
Checked on the state's own website on October 18, 2026. Rules change, so confirm with your DMV before your appointment.
What this means for you
Illinois does not collect a log. The parent comes to the license office with the teen to give consent and certify the hours, or completes a notarized Affidavit/Consent for Minor to Drive. The 50-hour log, DSD X 152.4, is only for your convenience.
