State log

14 states have their own log form

These states and D.C. publish their own log, with a row for each drive. Keep the record as you drive, then copy your drives onto the state's form before the appointment. Minnesota says it will not accept a log on any other document, so the copy matters there.

Signed in person

10 states have the parent sign at the DMV or school

In these states the parent signs for the hours at the DMV office or the driving school, so there is nothing to print at home. Some offer a notarized form for a parent who cannot come. Keep your own log so you know the hours are done before you go.

Any log

13 states accept any log

These states name no required log form. Any log works: a notebook, a spreadsheet or an app. A parent certifies the hours, often on the license application or the permit. Some states offer an optional log you can use.

Whatever your state asks for, you need the hours first. GoTime logs every drive with its day and night time, and exports the log as a PDF or CSV when you need it. In states with their own form, GoTime Pro fills it in.

Frequently asked questions

Does my state require an official driving log form?

Only 14 places do: 13 states and Washington, D.C. have their own log form with a row for each drive. In 14 more states a parent signs a state form that states the total hours, and in 10 the parent signs at the DMV counter or a driving school issues the form. The other 13 states name no required log form, so any log works. Find your state in the table above.

What is the difference between a driving log and a certification form?

A driving log has a row for each drive, with the date and the time spent driving. A certification form is a signed statement that the teen drove the required hours, with no rows. Many states ask only for the certification, but you still need a record of the hours to sign it honestly.

Can I use a driving log app?

Yes, in every state, to keep the record of each drive. Where any log works, the app's log is enough. Where the parent signs a form, the app's log is the record behind the signature. Where the state has its own log form, copy your drives from the app onto that form. Minnesota, for example, says it will not accept a log on any other document.

Which states need the form notarized?

In Arizona, Florida, Hawaii, Mississippi, Nevada, Ohio, Oklahoma, Pennsylvania and Rhode Island, the parent's statement must be signed before a notary or sworn before DMV staff. Arizona also accepts an MVD agent in place of a notary.

How does GoTime help with a state form?

GoTime logs every drive with its day and night time, and exports the log as a PDF or CSV, free. If your state has its own log form, copy your drives onto it from that record. GoTime Pro fills in the official form for you in states that require one. If your state asks for a signed statement, the log is the proof behind it.

How current is this information?

We read every form on the state's own website in October 2026. States change their hours and forms, so confirm with your DMV before your appointment.
